kpa100 top cover panel screws

I'm on page 43 of the KPA100 manual, and I find that the three threaded holes to secure the top cover panel are not deep enough for the black 3/16" pan head screws.  The cover is loose.  It works fine if I add a lockwasher under the screw heads.  I can't see anything amiss.  Is there any chance these weren't tapped deep enough? Any thoughts?

You sure you used the right screws? I built my KPA100 8 years ago, and did not have any problem securing the top cover panel.

Any debris any the screw holes?
