The ARRL has the 2011 list available at
http://www.arrl.org/files/file/DXCC/2011e%20DXCC%20Current.pdf

You can Google for DXCC Countries List and find other sources.

Beat me to it :) This one is more complete than the ARRL DXCC list,
which mostly just lists the primary prefix(es) for each entity.

If you want a complete list of all prefixes see the source:
    http://www.itu.int/online/mms/glad/cga_callsign.sh?lang=en

The list does not break out countries within countries (e.g., KH#,
KL#, KP# in the US callsign block) but it does give a complete list
of all prefixes assigned to a given country.
